Account Opening Documents

Status Enquiry and Consent Form (non-UK customers)
This form will be sent to your previous bank to confirm your residential address. Please remember to include this with your Account Application Form.

Standing Order Mandate
This form may be used for initiating regular payments or for changing an existing standing order from your Sterling or Direct Current Account only.

Salary Mandate & Account Switching
Like to deposit your salary into your Citibank current account and/or change your Direct Debits and standing orders to your Citibank current account? Simply complete our Account Switching Form (PDF) and we’ll manage the whole process for you.

Bill Payment
You can set up your bills for payment via the Internet using Citibank Online Banking. Alternatively you can pay bills over the phone using our touch-tone banking system and your Telephone PIN. Once you have returned your completed form to us, we will send you your own customised payee list card.

Bank Account Enquiry
You can obtain full details of Direct Debits and Standing Orders made from your previous account by sending this form to your previous bank/building society. Use the information you receive to transfer all your Direct Debits and Standing Orders to your Citibank Account.

Standing Order Cancellation
Please use this form to cancel any Standing Orders from your previous account. You should have instructed Citibank to make the payments from your Citibank Account before you cancel. Send the completed form to your previous bank/building society.

Direct Debit Mandate Request
Send one of these forms to everyone you pay by Direct Debit. They may need to send you new mandates, which you can complete with your Citibank Account details. Some Direct Debit originators may accept your request over the telephone if you prefer.

Direct Debit Cancellation Request
When you have received all the Direct Debit mandates in response to your Direct Debit Mandate requests, fill them in and return them to the organisations or persons you are paying. Send the completed form to your previous bank/building society.

You should have sufficient funds in your old account to cover any final payments being made from that account. Before you close your account check your old statements for quarterly, half-yearly or annual Standing orders and Direct Debits.
