This form has been created to facilitate the transfer of your entire investment portfolio to an alternative organisation, (referred to below as the "counterparty"). i This may be the bank or investment broker you will be transferring your portfolio to.   If you wish to transfer your entire investment portfolio to a single counterparty, you need only complete this form once.

Should you prefer to transfer your portfolio in parts, or to sell (rather than transfer) your portfolio, either in part or in its entirety, please contact your Relationship Manager or CitiPhone, who will be able to help with these and any other queries you may have.

Please note that, for your security, once we have received the form, we will need to verify your identity. To complete this process, we request that you enter the mobile number and e-mail address you have registered with Citi and we will call you back to confirm your details.
